It looks (from Alexa) like Phorm are currently using the domain PhormLabs.com to test OIX in the UK.

This version of Dephormation adds blocking & detection for the OIX domain PhormLabs.com, and optional link warnings for the OCSLab (Phorm Moscow) domains;

Incidentally, re the bit in your notes concerning addons.mozilla.org, "Firefox 4 periodically asks AMO [add-ons.mozilla.org] for updated information about all installed add-ons, and uses that information to update the information displayed in the Add-ons Manager panel."
 
I wasn't overjoyed about that either, I object to anything auto-updating without telling me - but as a user rather than a developer, it appears to be easy to kill.
 
The usual "about:config" - go to "extensions.getAddons.cache.enabled", and toggle to "false". That, I assume, will knock it on the head for the lot.
